Informed Consent for COVID-19 Testing

Please carefully read the following informed consent:

a. I authorize this COVID-19 testing unit to conduct collection and testing for COVID-19 through a nasopharyngeal swab, as ordered by an authorized medical provider or public health official.

b. I authorize my test results to be disclosed to the county, state, or to any other governmental entity as may be required by law.

c. I acknowledge that a positive test result is an indication that I must continue to self-isolate to avoid infecting others.

d. I understand the testing unit is not acting as my medical provider. Testing does not replace treatment by my medical provider. I assume complete and full responsibility to take appropriate action with regards to my test results. I agree I will seek medical advice, care and treatment from my medical provider if I have questions or concerns, or if my condition worsens.

e. I understand that, as with any medical test, there is the potential for false positive or false negative test results.

f. I, the undersigned that, have been informed about the test purpose, procedures, possible benefits and risks. I have been given the opportunity to ask questions before I sign, and I have been told that I can ask other questions at any time. I voluntarily agree to testing for COVID-19.

TYPES OF COVID TESTS

  • Rapid COVID-19 Antigen Test
  1. This test is used to rapidly detect an active COVID-19 infection in a patient.
  2. Results are generally reported within 20 minutes to an hour at most locations.


COVID Clinic utilizes multiple antigen tests in order to make antigen testing more accessible. The antigen test administered to patient              will be one of three tests, but COVID clinic is unable to guarantee which of the tests will be utilized for a specific patient.

  • Rapid COVID-19 IgG/IgM Antibody Test
  1. This test is used to detect COVID-19 antibodies. A positive antibody test result suggests that the patient was recently infected by COVID-19 or has developed an immune response through vaccination.
  2. Results are generally reported within 1 hour at most locations.


*This DOES NOT test for a current COVID-19 infection.

  • Rapid COVID-19 + Flu Combo Antigen Test
  1. Three tests in one! This test is recommended for all individuals and especially those with symptoms.
  2. Used to rapidly detect COVID-19, Influenza A and Influenza B.
  3. Results are generally reported within 1 hour.

  • What is SARS-CoV-2?

    SARS CoV-2 is an acronym for severe acute respiratory syndrome which is a virus that causes the disease of COVID-19.

  • Does COVID Clinic test children?

    COVID Clinic accommodates patients of all ages and capabilities. Parental consent is required to test minors which can be completed in the online consent form.


    What happens if I miss my appointment?

    - If you have placed an order for a test and scheduled an appointment at our clinic but are unable to make it at your scheduled time, the site will continue to honor your original order, provided all necessary forms have been completed. A patient may visit the site at their earliest convenience.


    Can I pay on site?

  • What if I want to check for a previous COVID infection?

    COVID clinic offers a rapid antibody test that can detect IgG and IgM antibodies within an hour. The presence of antibodies may be indicative of a past infection.

  • What are antibodies?

    Antibodies are proteins detected by a blood test that can help us determine if your body has developed a response to a COVID-19 infection or vaccine. A positive antibody test could indicate that you have had a past infection with COVID-19 or have been vaccinated.

  • Is the test covered by insurance?

    Insurance companies may reimburse for COVID testing services. It is the patient’s responsibility to be aware of any requirements imposed by his/her insurance company to obtain reimbursement for COVID testing services. COVID clinic encourages patients to contact their insurance company for additional information prior to receiving testing services if the patient intends to seek reimbursement for the cost of the testing services. COVID Clinic will provide an invoice for its testing services. COVID Clinic does not guarantee that any patient’s insurance company will provide reimbursement. Please be advised that CIGNA will not reimburse its members for self-pay COVID Testing services.

  • Why do you charge for testing?

    We believe that a patient should be able to test whenever they want, with or without insurance. Many of our patients have been successful in paying for testing and then submitting their invoice to their insurance company for reimbursement.
